A large segment of the population has been using traditional and herbal treatments for many generations. Additionally, they are being employed in more and more ways in every nation on earth [1]. Seven hundred and ninety-five percent of the population in the developing countries relies on these traditional medicines for primary care [2,3]. Plants are the primary source of medications used to cure a variety of ailments in underdeveloped nations [4]. The majority of medications available today are also derived from plants, and the usage of plant-based products for pharmacological purposes is expanding [5]. The World Health Organization states that the greatest place to get a range of medications is from medicinal plants. The vocabulary used to describe regulatory status varies greatly between nations [2,6]. Aloe vera is one of many plants that have been widely and often utilized for many years due to its alleged therapeutic benefits [7,8,9].

Aloe vera is a perennial, drought-resistant succulent plant with its origins in Africa. There are 400 different species in the Aloe family. It features lance-shaped, rigid green leaves with a mucilaginous pulp in the center that is filled with clear gel. Long droughts may be endured by the plant thanks to its water-storing large leaves [10]. The leaves can withstand extremely hard conditions and have a remarkable capacity to retain water, even in very heated, dry areas. An orange-yellow sap leaks from the cut end of a leaf when it is cut. A clear, mucilaginous fluid that contains water and the component to keep the water in the leaf is revealed when the green skin of a leaf is removed. According to reports, the gel contains 99.3 percent water and the remaining 0.7 percent is made up of solids, primarily carbohydrates [10,11,12].

Various nutrients, including minerals, sugars, phenolic compounds, lignin, saponins, sterol, and amino acids are believed to be present in aloe vera plants. Additionally, several vitamins, including the crucial antioxidant vitamins A, C, and F, are said to be present. Choline, folic acid, and the B vitamins (B1, B2, and B3) are also present. B12 is also present in trace amounts [7]. Stress and inflammation are said to be significantly decreased by vitamin B complex and vitamin C. Amylase, lipase, and carboxypeptidase are just a few of the enzymes that aloe vera is said to possess. Additionally, it is reported to contain antibacterial, antiviral, and antifungal properties. Aloe gel is thought to boost the immunological system of the body [8,11,13].

According to reports, aloe vera has strong laxative, antibacterial, and analgesic properties [14,15]. According to reports, aloe vera also contains saponins, which are soapy compounds with general cleaning effects as well as antibacterial and anti-carcinogenic qualities [13]. A molecule similar to aspirin with anti-inflammatory and anti-bacterial characteristics is also said to be present in aloe vera, along with campesterol, F2 sitosterol, lupeol, and other compounds [7,14,15,16].

Burns, allergic reactions, rheumatoid arthritis, rheumatic fever, acid indigestion, ulcers, diabetes, skin diseases, dysentery, diarrhea, piles, and inflammatory conditions of the digestive system and other internal organs, such as the stomach, small intestine, liver, kidney, and pancreas, are just a few of the conditions that the bioactive compounds from aloe vera are said to be very effective in treating. [16,17,18].

Most of Uganda and East Africa regularly grow and utilize aloe vera. Many people in Uganda use the plant extensively as a medicinal plant [5].

It can be used as an adjuvant or full therapy, among other things [8]. Products made from aloe vera are utilized in a variety of ways, including as over-the-counter (OTC) pharmaceuticals, self-care treatments, home remedies, dietary supplements, health foods, functional foods, and many more. Despite the fact that this plant has been used traditionally for medical purposes or because it may be hazardous, it is unclear how many people use it and how they use it. [6,19]. It is unknown what percentage of the population benefits from this plant or is exposed to its hazardous effects. As a result, this study aims to ascertain the extent to which communities in Western Uganda’s Sheema District use aloe vera plants locally to treat illnesses [20,21,22].

Nopal is a big prickly pear cactus that is indigenous to dry regions of South and North America. It is traditionally consumed by Mexicans as food and used medicinally as a laxative, anti-inflammatory, and to treat high blood sugar and alcohol hangovers.

Nopal has a blood sugar-lowering function, but the precise mechanism is unknown, despite the fact that it contains significant levels of soluble fiber and pectin, which may impact how well glucose is absorbed. Although earlier research disputes fiber’s significance in lowering blood glucose levels in animal trials, it does not propose a substitute mechanism. 34 Opuntia extract (1 mg/kg body weight) used for 7 weeks in conjunction with insulin and then Opuntia extract alone allowed blood sugar levels to quickly revert to those of non-diabetic rats. In a recent animal experiment, diabetic rats treated with streptozotocin (STZ) were given liquid and filtered extracts of Opuntia streptacantha to see how they would react. 35 The extracts weren’t able to lower blood sugar levels. But when administered before an OGTT, it had an antihyperglycemic effect, indicating a potential mechanism involving inhibiting hepatic glucose outflow.

A mild to moderately positive effect on people with T2DM has been confirmed by a number of small (N = 732) published clinical trials (all carried out by the same research group) utilizing various forms and doses of Opuntia species.
