The spent peace lily bloom should be trimmed at the plant’s base, leaving only the bloom stalk.
A flower will not form in the same area as before, whether you cut the leaf off or not.

Is it the bloom stalk or the leaf stem you’re referring to?
The bloom stem should be removed so that nothing is left to become yellow.
That is why you must cut the bloom stalk all the way down to the base of the plant, where it originates.
If the leaf stem is turning yellow, cutting the bloom stalk off may cause damage to the leaves.
If the leaf stem appears to be damaged in any manner, it must be removed for the sake of the plant’s health. I sometimes have to take a leaf off because of the way the blossoms are positioned. Trimming the leaves of your peace lily will not harm it.
