How is plant dermatitis managed?
One of the most frequent ways that plants harm people is through plant dermatitis. There are countless plants that can lead to dermatitis, and they can be found everywhere. Some plants can be fatal if consumed topically. 83 Human dermatitis is mostly brought on by plants through one of four major mechanisms: delayed hypersensitivity, phytophotodermatitis, irritating or mechanical dermatitis, and rapid hypersensitivity.
Plants that are urticariogenic induce dermatitis on direct touch. Some plants contain stinging hairs that puncture the skin and release irritant plant poisons (e.g., stinging trees of Australia and stinging nettle). Others only need physical contact to trigger a hypersensitive reaction (e.g., strawberry, castor bean, and chrysanthemum). Hives, pruritus, and occasionally shock and anaphylaxis result from contact with these plants. 84 Cold compresses and antihistamines are used to treat uncomplicated urticaria. Systemic reactions can call for more thorough treatment.
Numerous plants can irritate the skin and cause dermatitis. One way that plants harm people is by causing simple mechanical irritation with their prickly hairs or spines. A blooming plant’s thorns or a cactus’ spines can pierce the skin and spread pathogens like Staphylococcus aureus and sporothrix, as well as other diseases. 84 Some plants create chemical irritants that can lead to dermatitis. Several plants, including the manchineel tree, Dieffenbachia, and Philodendron, produce toxins that irritate the body chemically. Papular and vesicular responses, erythema, and edema are examples of acute clinical signs. Bullae, pustules, and ulcerations may develop in severe cases. The elimination of the irritating stimuli is the main focus of treatment for mechanical and chemical irritants. This could involve removing thorns manually or thoroughly rinsing the area to remove chemical irritants.
When the skin is exposed to both a plant toxin and ultraviolet light, phytophotodermatitis develops. A dermatitis characterized by a painful, vesicular, erythematous rash lasting one to two weeks can be brought on by celery, citrus, Queen Anne’s lace, and the common fig. Hyperpigmentation may appear on healing skin. Because it only affects skin that has been exposed to sunlight, phytophotodermatitis can be distinguished from allergic contact dermatitis by the appearance of hyperpigmentation during the healing process. 84
The condition known as allergic contact dermatitis, which is a delayed hypersensitivity reaction, is brought on by cell-mediated immunity. Mango, cashew, poison oak, and poison ivy can all result in allergic contact dermatitis. Only individuals who have been exposed to and sensitized by the plant experience the reaction. Erythema, vesiculation, weeping, and pruritus will appear on exposed areas. The lesions disappear over a period of weeks with typically no coloration or scarring left behind. Both allergic contact dermatitis and phytophotodermatitis can be treated. Antihistamines and topical corticosteroids may ease symptoms in mild cases. Oral corticosteroids should be used in more severe situations. 84
Can plant-based rashes spread?
Poison ivy and other poison plant rashes are contagious but not transferable. However, it’s possible to contract the rash from plant oil that might have adhered to clothing, dogs, gardening equipment, and other items that came into touch with these plants. The plant oil remains until it is removed with water or rubbing alcohol (sometimes for years on end).
A person with poison ivy cannot spread it by scratching since the rash only appears where the plant oil has come into contact with the skin. If the rash develops gradually over time rather than all at once, it might appear to be spreading. However, this is either due to frequent exposure to contaminated objects or plant oil that has become trapped under the fingernails or because the plant oil is absorbed at various rates on different sections of the body.
